Reviews of services for the care of people with long-term conditions started in September 2012 and finished in March 2013. The review programme covered commissioning and provision of services for people with long-term conditions, looking particularly at care of people with chronic obstructive pulmonary disease (COPD), diabetes (adults and children), heart failure and chronic neurological conditions. The reviews included primary care, community-based teams, specialist teams and rehabilitation services for these groups. Primary prevention of long-term conditions, end of life care and diabetic retinopathy screening were not covered. Other than children with diabetes, the reviews covered services for adults only.
