Long Term Conditions

Reviews of services for the care of people with long-term conditions started in September 2012 and finished in March 2013. The review programme covered commissioning and provision of services for people with long-term conditions, looking particularly at care of people with chronic obstructive pulmonary disease (COPD), diabetes (adults and children), heart failure and chronic neurological conditions. The reviews included primary care, community-based teams, specialist teams and rehabilitation services for these groups. Primary prevention of long-term conditions, end of life care and diabetic retinopathy screening were not covered. Other than children with diabetes, the reviews covered services for adults only.

Report of the visits to all health economies are available below along with an overview report which summarises the findings of the review visits and makes recommendations for providers and commissioners.
